Though other medical data is obviously sensitive in nature, neural data crosses a few more personal boundaries, revealing intimate details about someone’s thoughts, emotions, behaviour and cognitive functioning. Researchers argue this elevates the privacy debate to a higher level, with many calling for neurodata to receive enhanced legal protections beyond conventional measures to safeguard autonomy, privacy and informed consent.

One other concern is that AI could inherit long-standing inequalities and biases in neurological research. Women and minorities have historically been underrepresented in many studies, leading to algorithms trained on these datasets producing less accurate predictions for these groups. When it comes to neurological disorders, early diagnosis will dramatically affect the outcomes, so biased predictive algorithms risk widening existing healthcare disparities rather than reducing them.

Perhaps the greatest challenge, however, is explainability and fixing the black box problem. Most of today’s most powerful deep learning models can predict disease progression or identify subtle imaging patterns without revealing how they reached those conclusions. In neurology, where diagnoses often influence not only life-changing treatment decisions, but how people manage their day-to-day emotions and behaviour, researchers argue that clinicians cannot just blindly accept AI recommendations without understanding the reasoning behind them.

Though AI indeed has the potential to transform neurological research and drug development, this can only happen if ethical governance evolves alongside technology. Protecting mental privacy, ensuring proportional representation, improving transparency and developing a more neurologically specific set of regulations will be just as important in improving outcomes from this technology. For in order to properly study the human brain, we must first earn its trust.
